THE PANDEMIC COVID-2019: IMMUNOLOGICAL FEATURES

Abstract

Вступ. COVID-19 – нове захворювання , яке стало викликом для імунної системи людини. Імуні механізми  були не готові для  захисту на рівні всіх природжених та набутих механізмів. Лише мобілізація імунної системи, її «навчання» можуть дати бій вірусу SARS Cov-2.

Метою роботи було аналіз доказових досліджень щодо імунологічних особливостей на  рівні природжених та набутих механізмів, різних типів реагування імунної системи людини  на COVID-19.   

Методи. Використано аналітичний прийом доказовості оцінювання імунологічних методів щодо функціонального стану імунної системи хворих на COVID-19 у порівнянні з  практично здоровими людьми на основі доказових наукових пошукових баз PubMed, MedLine, SpringerOpen  The Journal of the American Medical Association (JAMA), The Cochrane Librar,  Medknow, Google Scholar, BioMed Central, EBSCO – ERIC, а також   джерел ВООЗ, euroCDC, CDC (Atlanta, USA), МОЗ України.

Результати. Імунна система відповідає на контакт з SARS-CoV-2 своїм природженим і набутим імунітетом.  Відомі порушення природжених механізмів імунної системи: рецепторний апарат(TLR,RIG, NOD, CLR, CDR)  антигенпрезентуючих клітин, активності НК-клітин, надмірна продукція хемокіну 10 тощо. Адаптивний імунітет характеризується надмірною активністю Т-хелперів 17, 1 типів з надмірним виділення прозапальних інтерлейкінів IL17,IL6,IL1 тощо та недостатністю функції Т-хелперів 3 типу. Активніть В-лімфоцитів проявляється продукцією більше 30 видів моноклональних актитіл. Особливості імунної відповіді на  SARS Cov-2 у людській популяції: 30 % - безсимптомні відповідачі (фізіологічна імунна відповідь), 55 % - легкий перебіг ( скоординована робота імунної системи) , 10 %- середньо-тяжкий перебіг  ( дефіцит імунної системи, гіперактивність імунної системи) , 5 % - тяжкий перебіг ( значний дефіцит імунної системи, цитокіновий «шторм», розбалансування імунної відповіді).

Висновок. Необхідне подальше вивчення імунних механізмів для оцінки формування специфічного імунітету, попередження імунозалежних  ускладнень , що дасть можливість  створити  вакцини до SARS-Cov-2, специфічні імуноглобуліни, моноклональні антитіла.
